IMPORTANT! Reporting Send Offs (Red Cards)

For send offs (red cards) please do the following for the appropriate league.


For Youth and Amateur games:

  • retain the player's card
  • complete the 24-hour Send Off Report for Youth or Amateur games
  • mail both the player card and the Send Off Report to the appropriate league authority as determined by contacting your assignor

For High School and Middle School games:

In addition, the Incident Report form is required to be completed in the event of any disqualification, penalty for flagrant unsportsmanlike conduct, "bench fouls" or other misconduct of a serious nature committed by anyone associated with a contest. It is also our expectation that you vigorously enforce all rules related to player and coach behavior.

The Incident Report form may also be used to report unsafe conditions of a facility, lack of cooperation or concern, or other incidents which in your opinion merit the attention of the Commissioner.
