Referee Assignment Availability Form

Instructions

Fill out this form to provide your availability to officiate for leagues that the SFSRA assigns. The SFSRA will notify you if you are assigned to any games.

Required fields are indicated with a red asterisk (*).

By completing this section, I certify that I am a currently licensed USSF Referee at this grade.

San Francisco High School and Middle School

By checking any of the following boxes, I certify that I meet the following requirements:

  • I am available on the selected weekdays for 3:45 PM kickoffs for High School and 3:30 PM for Middle School.
  • I have at least 3 years of experience officiating Under-16 youth soccer or older.
  • I am at least 18 years of age.
  • I have knowledge of high school soccer rules.
  • I will take or have recently taken a test concerning knowledge of differences in the rules applicable to high schools.
  • I will take or have recently taken a written test of the laws of the game.
  • I will participate and pass a fitness and agility test.
  • I will file a referee game report within twenty-four hours after the game.

Peninsula Athletic League (High School)

By checking any of the following boxes, I certify that I meet the following requirements:

  • I am available on the selected weekdays for 3:00 PM kickoffs.
  • I have at least 3 years of experience officiating Under-16 youth soccer or older.
  • I am at least 18 years of age.
  • I have knowledge of high school soccer rules.
  • I will take or have recently taken a test concerning knowledge of differences in the rules applicable to high schools.
  • I will participate in a "fun run" in either June or August.
  • I will participate and pass a fitness and agility test.
  • I will file a referee game report within twenty-four hours after the game.
