Instructions
Use of Forms
All of the following forms and materials are for use with USSF-affiliated youth, amateur and professional leagues. This includes games played under CYSA-North and CSAN.
 
Required Software

The forms and materials require software as noted in (parentheses), including Microsoft Word, Microsoft Excel, Microsoft PowerPoint, Adobe Reader and Stuffit Expander. Microsoft products must be purchased. All others may be downloaded for free. Click the product name above for more information.

 
Notes

If you are unable to download a file by clicking its link, try following the steps below.

Warning messages, dialog boxes and images may vary across computer systems, but the process is similar no matter what system you have.

 
Step 1

Macintosh: Click and hold the mouse button on the link of the file that you want to download.
Windows: Click the right mouse button.

The following dialog box will appear. Select "Save this Link as..."

 
Step 2

If this warning appears, click on the button "Pick app". Another dialog box will appear.

 
Step 3
Navigate through your system to locate the appropriate application. In this case, Microsoft Word. Click "Open".

Your browser is now configured to download and open any files with the filename extension with the application you have selected.

 
Troubleshooting
1. I cannot download the file.
Try the Instructions Steps 1-3 above. If that fails, see Question 5 below.
 
2. I click the link to download a file, and a warning message appears telling me that I have started to download a file named "XYZ". What do I do?
Don't panic, just click "Save file" to download into a location on your computer you select, or "Pick app" to pick an application to open the file. See Instructions above.
 
3. I click the link to download a file. The file downloads to my computer, but I cannot open the file.

Install the necessary software, and open the file, using the correct program:

 
4. When I click one of the links, my browser window fills with random text characters. What is going on?

Your web browser might not be configured to recognize files with certain "extensions" (the last three letters of the filename). For example ".doc" is the extension for a Microsoft Word file. You need to do two things:

  1. Install the required software (See Question 3), and
  2. you need to "teach" your browser to recognize these extensions. See Instructions above for details.
 
5. I have read the entire section on Instructions, following every single step, as well as Troubleshooting, and I have even asked my child to figure it out for me, but I still cannot download/open the file. What else can I do?
