Forms and Certificates for Use at Clinics

USSF Registration Form. See registration details on CNRA.net.
Physical Fitness and Written Examination Results Form (Microsoft Excel): Certificate for recording testing results for referee renewal and upgrade.
Written Test Answer Sheet Form (Microsoft Excel): For referees taking multiple-choice written tests for renewal and upgrade.
Clinic Feedback Form (Microsoft Word): For students to provide written feedback to Instructors about the instruction they have receive in a clinic. Instructors use feedback to improve their skills and methods.
New Referee Welcome Letter (Microsoft Word): For distribution to new referees at the end of Entry Level Referee Clinics. Sample from District 1.

Clinic Administration Tools

Referee Clinic Coordination (Microsoft Word): Outline of responsibilities of League, DDI and Referee Candidates. Sample from District 1.
Clinic and Book Request Form (Microsoft Word): Organization requesting a clinic completes this form, then sends it to the DDI for processing. Sample from District 1.
USSF Referee Administrative Handbook 2005 edition (Adobe Reader, PDF): Contains everything the referee needs to know off the field of play for professional development. Includes upgrade and renewal requirements.
Referee Clinic Roster (Adobe Reader, PDF): Lead Clinic Instructor submits this roster with referee checks and referee registration forms to State Director of Instruction. Modified from the CNRA original to include email address. Sample from District 1. Generated from a FileMaker Pro database that also tracks physical fitness and written testing results, and features email list management. Contact web@sfsra.org for more information on the database.

Entry Level Referee Clinic Materials

Entry Level Referee Clinic materials may be downloaded from the USSF site. However, they list each item separately and it can be a hassle to download them all. For your convenience, the SFSRA has provided the Entry Level Referee Clinic Lesson Plans as a single downloadable file. The Presentation Slides are available only from the USSF site due to their large file size.

Many Instructors have found that the USSF Presentation Slides are crammed with too much information, making them ineffective learning tools. It is recommended that Instructors carefully select only a few slides, or better yet create their own slides to suit their instruction style. Experienced Instructors do not rely upon slides as the sole instruction tool or method.

Other Instruction Materials

USSF Instruction and Training Materials: On the United States Soccer Federation web site for Referees. Includes Assignor course, Grade 9 Recreational Referee course, Grade 10 Assistant Referee course, Small-Sided Training course, Supplemental Instruction Materials, Intermediate Training course and State Level Training course.
USSF Instructors Casebook - Analysis of Match Situations (Adobe Reader, PDF, PowerPoint, Stuffit Expander): This is an advanced topic meant for officials working higher level matches. It contains a PowerPoint presentation, Adobe Reader, PDF memo and casebook, and a readme file.
