Registration

USSF Registration Form. See registration details on CNRA.net.
USSF Referee Administrative Handbook (Adobe Reader, PDF): Contains everything the referee needs to know off the field of play for professional development. Includes upgrade and renewal requirements.
AYSO/USSF Cross-certification Form (Adobe Reader, PDF): For AYSO Referees who want to become certified as USSF Referees. See form for complete instructions and requirements.

Game/Misconduct/Supplemental/Send-Off Reports and Substitution Passes

CYSAN Send Off Report (web-based) for all CYSA District 1 Youth Matches: This web-based form validates the data submitted by the referee. The referee and appropriate authority, if available, receive an email with the official CYSAN Send Off Report attached as a PDF, but pre-filled with all the data submitted. The attached PDF can be saved, printed and mailed with the player's or coach's card to the appropriate authority.
CYSAN Send Off Report (PDF): The official report to use for reporting misconduct in CYSAN (youth) games. Can be filled out on a computer and printed. The only drawback is that the data you enter cannot be saved to your computer. Courtesy of Henrique Martins.
CSAN Send Off Report (Adobe Reader, PDF): (Courtesy of Greg Dunlap.) For reporting send offs (red cards) within 24 hours for all amateur leagues in California North. Can be filled out on a computer and printed. The only drawback is that the data you enter cannot be saved to your computer.
USSF Referee Game Report and Supplemental Report (Word): For reporting a summary of the match, plus misconduct and unusual incidents. Reports can be saved electronically.
USSF Referee Game Report and Supplemental Report (Adobe Reader, PDF): Same as above, but uses Adobe Reader. For viewing and printing only. Cannot be filled out on a computer.
